RN Case Manager – Simplified

• Position: RN Case Manager
• Shift: Monday–Friday, 8:00 AM–5:00 PM
• Hours: 40 hours/week
• Contract: 13 weeks

Requirements

• RN Case Management experience: Minimum 2 years recent experience
• Must have experience in:
  • LTAC or STACH
  • MCG
  • Discharge planning in acute-care settings
  • Strong collaboration with the Interdisciplinary Team (IDT)
• Texas RN license or Compact/Multi-State RN license
• BLS through American Heart Association (AHA) — must be attached to submission

Main Responsibilities

• Assess patients and coordinate their care.
• Create and implement care plans.
• Coordinate discharge planning.
• Ensure proper use of healthcare resources.
• Work with doctors, nurses, social workers, and other healthcare professionals.
• Advocate for patients and consider their individual needs and preferences.
• Help improve patient satisfaction, continuity of care, and cost effectiveness.
• Perform utilization review and case management.
