Job Description & Requirements

REQUIRED EXPERIENCE

  • Position: RN – Nurse Case Manager.
  • Minimum 1 year of experience in case management, social work, and discharge planning – Required.
  • Cerner EHR proficiency – Mandatory.
  • Previous rural healthcare experience is strongly preferred.
  • Must be comfortable coordinating patient care in a resource-limited healthcare environment.

SCHEDULE

  • Shift: Days.
  • Hours: 8:00 AM–4:30 PM.
  • Schedule: 5 × 8-hour shifts per week.
