Seeking candidates with recent experience as a Case Manager, Care Manager, or Discharge Planner within an acute care hospital environment. The role involves coordinating care for acutely ill patients with complex medical conditions and discharge planning needs. Requirements: 2+ years of Care Management or Case Management experience in an acute care setting required.
830-5 M-F
Required: RN License, BLS
Required Education:
-ADN or ASN from an accredited school of Nursing.
Preferred Education:
-BSN or MSN from an accredited school of Nursing.
