You will promote the use of evidence-based practice through protocols and procedures. You will assess the patient's status through interview, observation, and physical assessment of actual problems and outcomes. You will develop an individualized plan of care using a collaborative approach with the patient and health care team. You will use appropriate interventions identified in the plan of care to achieve expected patient outcomes. You will systematically evaluate and document the patient's progress toward expected outcomes and goals on an ongoing basis throughout the shift. You will modify the plan of care based on evaluations. You will complete required documentation as appropriate, including patient chart and event reports. You will demonstrate professional accountability for your practice by seeking resources and continuing self-development. You will demonstrate technical knowledge of equipment and troubleshoot when problems arise. You will acquire and maintain current knowledge and skills in evidence-based nursing practice. You will promote the professional development of yourself and team members. You will share clinical knowledge through teaching and in response to the learning needs of others. You will demonstrate a practical problem-solving approach to patient moving and handling issues when applicable. You will demonstrate skills to promote patient and staff safety through appropriate use of lift equipment when applicable. You will possess knowledge and skills necessary to obtain adequate lab specimens for testing. You will perform patient bedside lab testing and associated quality control in compliance with medical provider orders and hospital procedures. You will coordinate the interdisciplinary care of the patient through collaboration with team members and utilize additional resources when necessary. You will analyze clinical situations in a systematic way and act to obtain resources or correct problems to meet or exceed expected outcomes or patient needs. You will recognize the need for assistance and utilize resources in a timely manner. You will demonstrate ownership and persistence to ensure problems are resolved in the best way possible. You will assume assignment of non-complex and some complex patient management with some turnover in assignment. You will consistently prioritize work based on changes in patient condition and unanticipated interruptions. You will identify and utilize team resources to solve identified problems or needs. You will collaborate with the health care team in organizing tasks and activities for the patient’s care from admission to discharge according to the plan of care. You will work with other team members in delegating tasks and activities appropriately. You will provide goal-focused shift report. You will advocate for patients with physicians when necessary. You will utilize communication and teamwork strategies aimed at promoting high team performance and achieving optimal patient outcomes. You will choose an appropriate communication style when interacting with others. You will give and accept constructive feedback from peers.
