The focus is on creating a caring environment aimed at healing and wellness. Nursing practices are based on evidence, current research, and established policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and other disciplines. Responsibilities include assessing patient needs and documenting comprehensive assessments in line with scope of practice. Consideration is given to patient culture, age, and developmental level. Identifying patient risk factors and presenting complaints is essential. Health information is obtained from families or others as necessary. Objective and subjective data are collected for thorough evaluation. Initial admission assessments are verified if performed by other team members. A problem list is developed, prioritizing patient needs based on inter-professional team input. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family according to the medical plan of care. Patient care therapies and procedures are fully implemented with clinical autonomy within the established scope of practice. Procedures, treatments, and processes are explained to the patient and family. Initiatives are taken to alleviate patient stress and discomfort. Recognizing and responding to changes in patient condition is vital. Ongoing assessments and responses to interventions are documented according to hospital policy. Delegation of tasks is performed appropriately, overseeing team members' patient care activities. Monitoring the accuracy and completeness of delegated care is crucial. Educational needs of patients regarding prioritized problems are identified and addressed. Facilitating the discharge process involves timely execution of discharge orders and coordination with case management. Input into patient/nurse assignments is provided considering available staffing and patient acuity. Participation in unit-based activities supports organizational values and priorities. Promoting a positive group dynamic and healing environment through words and actions is essential. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, and research. Active participation in quality improvement activities and core measures is encouraged. A holistic perspective regarding patient care is maintained.
