Implement and monitor individualized patient care plans. Evaluate outcomes and adjust treatment to optimize patient safety and recovery. Administer physician and nursing orders promptly. Maintain compliance with regulatory standards and advance quality improvement initiatives. Lead discharge planning efforts. Collaborate with patients and the interdisciplinary team to ensure safe transitions and effective post-acute care planning. Manage a patient population primarily consisting of high-risk cardiac surgeries. Provide care for open heart recovery, heart transplants, LVAD implants, ECMO, and more. Utilize monitoring techniques, including Swan Ganz Catheters and CCO monitoring. Serve in various related units as necessary, based on patient needs.
