Assist patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborate with interdisciplinary teams to coordinate patient care, including patient transfer, discharge, referrals, and spiritual/psychosocial support needs. Evaluate learning needs of patients and/or families and provide education appropriate to age, culture, condition, and circumstances. Advocate for the physical and emotional well-being of the patient.
