Registered Nurse (RN) Overview
- Supports a team-based primary care model focused on delivering high-quality patient care and care coordination
- Builds strong relationships with patients and caregivers while supporting preventative and chronic care management initiatives
- Coordinates patient care plans and clinical services to support positive health outcomes
- Provides patient education related to chronic conditions, treatment plans, medications, and preventive care
- Collaborates with interdisciplinary care teams to ensure seamless care delivery across the healthcare system
- Participates in care team meetings and supports clinical decision-making through patient outreach and care coordination activities
- Utilizes strong communication, problem-solving, and organizational skills to support patients and improve healthcare experiences
- Delivers high-touch patient care through proactive outreach, symptom management, education, and care plan monitoring.
Job Duties
- Provide telephone support and triage for patients
- Schedule problem visits and nurse visits as appropriate
- Offer symptom management guidance
- Support prescription refill requests
- Answer patient follow-up questions
- Provide patient education regarding chronic conditions
- Conduct patient education in person, by phone, or in group settings
- Monitor and coordinate patient care plans
- Build relationships with patients and caregivers
- Support prevention-focused care initiatives
- Assist with emergency department and hospital diversion efforts where appropriate
- Provide direction to Medical Assistants regarding procedures, telephonic support, and patient instructions
- Conduct medication authorizations
- Conduct testing authorizations
- Deliver exceptional patient service and care
- Perform home visits as needed
- Participate in care team meetings
- Perform other duties as assigned
