Registered Nurse (RN) Overview

  • Supports a team-based primary care model focused on delivering high-quality patient care and care coordination
  • Builds strong relationships with patients and caregivers while supporting preventative and chronic care management initiatives
  • Coordinates patient care plans and clinical services to support positive health outcomes
  • Provides patient education related to chronic conditions, treatment plans, medications, and preventive care
  • Collaborates with interdisciplinary care teams to ensure seamless care delivery across the healthcare system
  • Participates in care team meetings and supports clinical decision-making through patient outreach and care coordination activities
  • Utilizes strong communication, problem-solving, and organizational skills to support patients and improve healthcare experiences
  • Delivers high-touch patient care through proactive outreach, symptom management, education, and care plan monitoring.

 

Job Duties  

  • Provide telephone support and triage for patients
  • Schedule problem visits and nurse visits as appropriate
  • Offer symptom management guidance
  • Support prescription refill requests
  • Answer patient follow-up questions
  • Provide patient education regarding chronic conditions
  • Conduct patient education in person, by phone, or in group settings
  • Monitor and coordinate patient care plans
  • Build relationships with patients and caregivers
  • Support prevention-focused care initiatives
  • Assist with emergency department and hospital diversion efforts where appropriate
  • Provide direction to Medical Assistants regarding procedures, telephonic support, and patient instructions
  • Conduct medication authorizations
  • Conduct testing authorizations
  • Deliver exceptional patient service and care
  • Perform home visits as needed
  • Participate in care team meetings
  • Perform other duties as assigned
