RN Case Manager — Advocate Trinity Hospital
Department: Inpatient Care Management
Shift: 8-hour day shift
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 40 hours/week
Facility
- Beds: 205
- Level: II NICU
- Trauma designation: None
- Primarily serves an underserved population, including adults and geriatrics.
- Patient population may include complex social situations such as unhoused and low-income patients.
Position Requirements
- Minimum experience: 3 years of acute-care case management experience
- First-time travelers: Accepted
- Education: ASN or higher accepted
- Certifications: BLS
- EMR: Epic
- Referral system: Careport
- Strong assessment and critical-thinking skills required.
- Must be comfortable:
- Working with physicians and multidisciplinary teams
- Navigating different insurance plans
- Finding community/facility resources
- Asking questions and advocating for patients
- Participating in facility initiatives
- Teamwork and interpersonal skills are emphasized.
Patient Assignment
- Typical caseload: approximately 14–20 patients
- Potential assignments include:
- Med-Surg
- PCU
- LDRP
- ICU possible, but not expected
- May be assigned to a specific unit but could float to other inpatient units as needed.
