Registered Nurse (RN) Team Lead - Skilled Nursing Facility
Job Summary
This role is for a dedicated Registered Nurse (RN) Team Lead within a 120-bed Skilled Nursing Facility (SNF) located in Montour Falls, New York. The facility is structured with three units, each accommodating up to 40 residents and managed by an RN.
The Team Lead will oversee patient care on a unit, collaborating with a care team typically consisting of 1 RN, 2 LPNs, and 3-4 CNAs for 40 residents during the assigned shift. This position requires strong leadership, clinical expertise, and the ability to adapt to varying patient needs and staffing requirements.
Key Responsibilities
- Provide direct patient care, including comprehensive assessment, monitoring, recording, and reporting of patient conditions.
- Demonstrate strong time management, critical thinking, delegation, and prioritization skills in daily operations.
- Collaborate effectively with the interdisciplinary care team to ensure coordinated and holistic patient care.
- Act as a patient advocate, ensuring ethical and professional practice within the scope of nursing.
- Promote evidence-based practice (EBP) and best practices to optimize patient outcomes.
- Maintain clinical competence specific to the care area and complete all required organizational education.
- Document patient care accurately and thoroughly in the Electronic Medical Record (EMR).
- Participate in unit councils and staff meetings to contribute to continuous quality improvement.
- Float to other units as staffing needs permit to support overall facility operations.
- Be prepared to function in an LPN Med Cart position as needed and staffing permits.
Required Qualifications
- Active Registered Nurse (RN) license in the State of New York.
- Ability to function in an LPN Med Cart position as needed.
- Physical ability to lift up to 50 lbs.
- Ability to sit or stand for long periods.
- Proficiency in operating equipment required for the job.

- Clinical Skills: Patient assessment, vital sign monitoring, medication administration (including LPN Med Cart duties if required), wound care, EMR documentation, evidence-based practice.

Shift Details
- Assignment Dates: September 13, 2026, to December 12, 2026 (13-week contract).
- Shift Type: Evening Shift
- Hours: 3:00 PM - 11:30 PM (8-hour shifts)
- Days per Week: 5 days per week (40 hours total)
- Weekend Rotation: Required every other weekend.
- Holiday Rotation: Required every other holiday.
