Registered Nurse (RN) Team Lead - Skilled Nursing Facility

Job Summary

Join our dedicated healthcare team as a Registered Nurse (RN) Team Lead in a 120-bed Skilled Nursing Facility (SNF) located in Montour Falls, New York. This facility is comprised of three units, each accommodating up to 40 residents, and is supported by an RN Manager on each unit. As an RN Team Lead, you will play a pivotal role in delivering high-quality patient care and coordinating the efforts of a multidisciplinary team.

This position involves direct patient care, including comprehensive assessments, monitoring, recording, and reporting of patient conditions. You will lead a care team with a typical staffing ratio of 1 RN, 2 LPNs, and 3-4 CNAs per 40 residents, ensuring efficient and compassionate care. We are seeking a professional who excels in time management, critical thinking, delegation, and patient advocacy, committed to promoting evidence-based practices.

Key Responsibilities
  • Provide direct patient care, including thorough assessment, monitoring, accurate recording, and timely reporting of patient conditions.
  • Lead and coordinate the care team, demonstrating strong skills in time management, critical thinking, delegation, and prioritization.
  • Collaborate effectively with the interdisciplinary team to ensure comprehensive and coordinated patient care.
  • Act as a patient advocate, ensuring their needs and preferences are respected and addressed.
  • Practice ethically and professionally within the scope of nursing practice.
  • Promote and integrate evidence-based practices (EBP) and best practices into daily patient care.
  • Maintain clinical competence specific to the care area, including completion of all required organizational education within specified timeframes.
  • Communicate effectively with patients, families, and the healthcare team.
  • Document patient care accurately and thoroughly in the Electronic Medical Record (EMR).
  • Participate actively in unit councils and staff meetings to contribute to unit improvements and team cohesion.
  • Demonstrate flexibility by floating to other units as staffing needs permit.
  • Be prepared to function in an LPN Med Cart position as staffing needs require.
Required Qualifications
  • Current Registered Nurse (RN) license.
  • Ability to function in an LPN capacity (Med Cart) as needed.
  • Ability to lift up to 50 lbs.
  • Ability to sit or stand for long periods.
  • Must be able to physically operate all equipment required for the job.
