Licensed Vocational Nurse (LVN) - Skilled Nursing
Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) for a skilled nursing facility (SNF) in Lake Isabella, California. This role involves providing direct patient care to residents within a supportive and collaborative environment, working under the supervision of a Registered Nurse and Director of Nursing.
The LVN will be responsible for assessing resident conditions, developing and implementing nursing care plans, and ensuring the continuity of care between shifts. Key daily activities include medication administration, pain management, accurate documentation, and supervising Certified Nursing Assistants (CNAs) and other supportive staff members to maintain high standards of resident care and safety.
Key Responsibilities
- Assess residents' conditions, identify nursing needs, and develop appropriate nursing actions and care plans.
- Administer medications following the eight (8) medication rights, actively working to reduce potential medication errors.
- Perform waived testing (point-of-care testing) according to established policies and procedures, interpret results, and take appropriate action.
- Document changes in residents' conditions, nursing care provided, and progress notes accurately and in a timely manner.
- Notify physicians of changes in resident condition and ensure follow-through until appropriate action is taken.
- Accurately transcribe and implement physician's orders in a timely fashion.
- Provide thorough shift reports, including participation in walking rounds, to ensure continuity of care.
- Implement individualized care plans based on assessments and goals established by the interdisciplinary care team.
- Supervise and delegate tasks to Certified Nursing Assistants (CNAs) and other supportive staff to achieve care plan objectives.
- Participate in resident and family teaching regarding care, treatment, and discharge planning.
- Maintain strict adherence to infection control procedures and supervise supportive staff in their compliance.
- Act as a resident advocate, protecting resident rights and addressing psychosocial, cultural, ethnic, and religious/spiritual needs.
- Participate in the orientation of new personnel and assist with performance evaluations as needed.
Required Qualifications
- Graduate of an accredited Licensed Vocational Nurse (LVN) program.
- Current Basic Life Support (BLS) certification.
- Ability to read, write, and communicate effectively in English.
Preferred Qualifications
- Proficiency in additional languages.
- Basic computer knowledge.
- Experience with Electronic Health Record (EHR) systems.
