Key Responsibilities

  • Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care in collaboration with the patient, family, and interdisciplinary team
  • Provide accurate, succinct, and timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy
  • Participate in the patient rehabilitation program, utilizing the nursing process to assess, plan, implement, and evaluate patient care with a focus on patient and family education
  • Initiate discharge planning from the day of admission and communicate pertinent changes in patient status to attending physicians, charge nurses, and care team members to support continuity of care
  • Provide direction, training, and supervision of Licensed Practical Nurses, Certified Nursing Assistants, and orientees, delegating appropriately and utilizing effective problem-solving techniques
  • Assume charge nurse responsibility following orientation period as requested

Required Qualifications

  • Active Registered Nurse (RN) license in the state of Alabama or applicable compact licensure
  • Demonstrated ability to perform nursing assessments and develop individualized care plans
  • Competency in supervising and delegating to ancillary nursing staff
  • Ability to document accurately and in a timely manner in accordance with facility policies
  • Commitment to patient and employee safety standards, including safe patient handling and infection control practices

Preferred Experience / Skills

  • Prior experience in an acute rehabilitation or inpatient rehabilitation setting
  • Experience participating in interdisciplinary team conferences and rehabilitation care planning
  • Background in charge nurse or leadership responsibilities
