- Schedule: Day Shift (5x8) | 8:00 AM – 4:30 PM
- Duration: 13 Weeks
- Start Date: ASAP
This role is ideal for MSWs with acute care hospital experience who enjoy working directly with patients and collaborating with interdisciplinary teams to ensure safe and effective discharge planning.
What You'll Be Doing:
- Working as a Case Manager/Discharge Planner in an acute care setting
- Managing a caseload of approximately 20–23 patients
- Supporting patients and families throughout their hospital stay and discharge process
- Collaborating with healthcare teams to provide exceptional patient care
Additional Details:
- Floating may be required to the Laguna facility
- Variable 5-day workweek based on departmental needs, including weekends
- Maximum of 7 days of RTO allowed at submission
- Adult patient population
