Ability to work with a diverse patient population including neonates, adolescents, adults, and geriatrics. Knowledge of drug and alcohol abuse treatment options, mental health, and crisis intervention. Description: We are seeking an experienced Social Worker to join our team. The role involves working closely with patients and families to facilitate care coordination and discharge planning. You will manage a caseload of up to 25 patients and may monitor multiple units on weekends. Weekend coverage is required every other weekend, along with holiday availability. You will participate in the development of treatment plans and lead family meetings. Familiarity with computerized documentation systems is essential for this position. Collaboration with interdisciplinary teams to ensure continuity of care is expected. Strong skills in patient education, advocacy, and community resource navigation are vital. Candidates must demonstrate effective communication and documentation practices. This role focuses on supporting patients through complex health challenges and promoting safe transitions to home or other settings.
