Connected Health is hiring a compassionate and dependable Licensed Practical Nurse (LPN) for one of our trusted partner facilities in Wakefield, Nebraska. This opportunity is based in a skilled nursing and assisted living setting where patient-centered care and teamwork are top priorities.
Schedule:
- Night Shift
- 3x12 schedule
- Rotating weekends
Responsibilities:
- Provide direct nursing care to residents in skilled nursing and assisted living
- Administer medications and treatments as ordered
- Monitor residents conditions and report changes appropriately
- Collaborate with nurses, providers, and care staff to ensure quality care
- Maintain accurate documentation and support resident safety and comfort
Qualifications:
- Active Nebraska LPN license required
- Minimum 1 year of skilled nursing experience required
- Strong clinical and communication skills
- Compassionate, reliable, and team-oriented mindset
