5 x 8 ’s
Shift: Days
9am-5:30pm with alternating weekends

Experience:

Required Work Experience: 1 year of Case Manager experience and 2 years of LVN experience
Experience required: Discharge Planning, Case Coordination, will be working closely with the Case Manager and Nurses.
Certs: BLS, LA County Fire Card
License: Active CA LVN License
