LVN Case Manager – Days | Los Angeles, CA

Join a leading healthcare system for a 13-week Case Management contract in Los Angeles! This is a non-bedside LVN role focused on discharge planning, care coordination, and patient placement.

Position Details

• Start Date: ASAP
• Contract: 13 weeks
• Schedule: 5x8s | 40 hours/week
• Shift: Days, 9:00 AM–5:30 PM
• Alternating weekends required
• No floating
• No on-call
• Up to 4 days RTO
• Local candidates accepted
• Client interview required

Pay Package

• $2,000/week gross
• $25/hr taxable
• $1,000/week lodging stipend for eligible travelers
• OT: $37.50/hr

Requirements

• Active California LVN License
• BLS
• LA County Fire Card
• 2+ years LVN experience
• 1+ year Case Management experience
• Experience with discharge planning and case coordination
• Experience coordinating placements, dispositions, and home care/comfort needs
• Strong communication and organizational skills
• Travel experience preferred, not required

Role Overview

Work closely with RN Case Managers and nursing teams to coordinate patient care and discharge needs. This role is not bedside and is best suited for an LVN with experience in care coordination, discharge planning, placements, and transitions of care.

Orientation: Structured training including online education and precepted orientation
