Job Summary:
Long Term Care (LTC) Registered Nurse (RN) position located in a post-acute care setting. The role involves providing patient assessments, medication administration, wound care, and ensuring quality care for residents in a 28-bed skilled nursing facility. The position requires collaboration with an interdisciplinary team and adherence to facility protocols.
Job Details:
- Schedule: Rotating shift, with shifts including days from 0600-1430, evenings from 1415-2245, and nights from 2230-0700. Candidates may be required to stay up to 4 hours past shift for overtime per union policies and may rotate between shifts to meet staffing needs. Weekly schedule includes days, evenings, and rotating weekends.
- Required Qualifications: Valid BLS (Basic Life Support) certification; Active nursing license; Minimum of 2 years of experience in a similar setting; Valid certifications and education applicable to the role; No IV certification required for Licensed Practical Nurses; Location requirements as applicable.
- Preferred Qualifications: Experience with Meditech electronic medical records; Prior experience in long-term care or skilled nursing facilities; Ability to work with residents diagnosed with dementia, COPD, CHF, DM, and HTN.
- Additional Information: The unit is a short-term rehab, long-term care, or skilled nursing facility with no locked memory units, featuring private rooms and a typical census of 23-28 residents. Interdisciplinary team includes physical and occupational therapists, with daily meetings. On-site pharmacy services are available, and facility orientation includes both general and shift-specific training.
