Job Summary:
Long Term Care Nurse Practitioner in a rehabilitation center and nursing home setting. The position involves providing short-term rehabilitative care, supervising certified nursing assistants, administering medications, performing rounds, and wound care. The assignment is scheduled to begin on October 5, 2026, with an anticipated duration through December 28, 2026.
Job Details:
- Shift: Nights, 12-hour shifts, with a minimum of 36 hours per week.
- Required Qualifications: Valid RN license, CPR certification, required skill in skilled care and orthopedics, experience in short-term rehabilitation and long-term care, at least 2 years of relevant experience, ability to perform rounds, medication administration, supervise CNAs, and wound care.
- Preferred Qualifications: None specified.
- Schedule: Night shifts from 7:00 PM to 7:00 AM, with a minimum of 36 hours per week.
- Additional Information: Floating to long-term care facilities may be required. Scheduling may include blackout dates from 09/07 to Labor Day. Candidates must have a permanent home address on the submission, and the SSN must be on the tax tab of the candidate's page but not included in emails or sub packets. Travelers must reside more than 50 miles from the facility. The position involves caring for a patient ratio of 1:17 in a facility with 17 beds on skilled nursing and 55 long-term care beds, serving short-term rehab patients and managing wound care if needed.
