    Job Requirements

    Education & Licensure

    • Valid Wisconsin RN or LPN license
    • High school diploma or equivalent

    Certifications

    • Current CPR/BLS certification
    • Current Wisconsin RN or LPN license

    Experience

    • Minimum of 1 year of LTC/SNF experience
    • Strong clinical skills and ability to provide compassionate, resident-centered care
    • Excellent communication and teamwork skills

    Additional Requirements

    • Resume required
    • Must be local/semi-local to Southwest Wisconsin for local contract consideration
    • Ability to work PM shifts, 2:30 PM – 11:00 PM
    • Flexible scheduling with 5–8 hour shifts
    • Every other weekend required
    • No on-call or callback requirements

    Job Responsibilities

    • Provide high-quality long-term care and resident-centered care in a skilled nursing facility
    • Administer medications and treatments according to physician orders and facility protocols
    • Monitor, assess, and document resident conditions and progress
    • Collaborate with physicians, nurses, CNAs, and other interdisciplinary team members
    • Maintain accurate documentation in the facility’s EHR
    • Follow infection control procedures and facility safety standards
    • Communicate changes in resident condition appropriately
    • Support residents' physical, emotional, and overall well-being
    • Maintain compliance with facility policies and state and federal regulations
