The worker will provide crisis risk assessments for both adults and children. The job requires collaboration with an interdisciplinary team to ensure continuity of care. Ideal candidates will have experience in care coordination and case management. Documentation of patient information is essential for effective care facilitation. Knowledge of Medicaid and Medicare is beneficial. Candidates must be available to work every other weekend and some holidays.
