Responsibilities
- Administer medications and treatments per physician orders and care plans
- Monitor resident conditions, perform routine assessments, and document findings
- Assist residents with activities of daily living, promoting dignity and independence
- Provide basic nursing care, vital signs, wound care, and routine procedures per policy
- Maintain accurate documentation in the electronic health record and communicate changes to the supervising RN
- Collaborate with interdisciplinary team to support resident goals and safety
Requirements
- Active LPN license to practice (PA licensure preferred/highly valued; eligible candidates considered)
- Current CPR certification
- Minimum 1 year of practical nursing experience in a long-term care or skilled nursing setting
- Ability to work flexible shifts and travel per assignment
- Proficiency with electronic health records and documentation
- Strong communication and teamwork skills; ability to work effectively with diverse resident populations
