Responsibilities
- Provide direct nursing care to residents in a long-term care context, under the supervision of a registered nurse or physician
- Administer prescribed medications and treatments, monitor responses, and document findings
- Conduct patient assessments, measure vital signs, and report changes in condition promptly
- Assist with activities of daily living and support rehabilitation and restorative care as outlined in care plans
- Develop, implement, and update individualized care plans; collaborate with an interdisciplinary team
- Maintain patient safety, infection control, and documentation standards; respond to emergencies as needed
- Be willing to float to multiple units within the facility to meet patient care demands
Requirements
- Active Licensed Practical Nurse license in New York state
- BLS (Basic Life Support) certification
- 1+ years of practical nursing experience in a long-term care or equivalent setting
- Experience caring for residents with dementia and related needs
- Ability to work 12-hour day shifts, three days per week, including every other weekend
- Must be willing to float between units as required
