Attend team and unit rounds. Utilize Allscripts, Careport, and Parachute applications for referrals and orders. Ensure all patients are notified of their discharge in accordance with time frames and documentation specified in departmental policies. Meet with all patients on the caseload a minimum of once per week, unless clinically inadvisable (e.g., patient too disorganized to tolerate). In such cases, meetings should be adjusted to accommodate the patient's needs. Provide and review discharge plans and aftercare referral forms with patients or families as indicated. Complete screenings, assessments, notes, and care plans within time frames in the EMR. Participate as a member of the interdisciplinary team. Develop comprehensive and safe discharge and aftercare plans with the team. Develop and maintain relationships with community agencies. Execute discharge plans, including referrals for home care and skilled nursing facilities.
