Knowledge of cultural issues is essential. The candidate must have experience with high-risk intervention patient vulnerability or safety risks. Experience in collaborating with internal and external team members is required. Candidates must be knowledgeable in care coordination, emergency department practices, and referrals to other resources and facilities. Familiarity with the Epic charting system is mandatory. Candidates are expected to manage an average caseload of 18-24 patients, which may increase during weekends. Traveling staff may be expected to float between nearby facilities. Refusal to float may lead to termination of assignment.
