Description
Key Responsibilities
Provide clinical social work services within an Emergency Department setting
Conduct psychosocial assessments and develop care plans for patients and families
Coordinate discharge planning and connect patients with appropriate community resources
Collaborate with interdisciplinary care teams to support patient outcomes
Document clinical activity and patient interactions using Epic EMR
May be required to float within the hospital or to nearby affiliate facilities on a per-unit basis
Required Qualifications
Minimum 2 years of clinical social work experience
Active LCSW or CSW license required (pending licensure not accepted)
BLS certification required (AHA Traditional - RQI not accepted)
Experience in an Emergency Department setting required
Epic EMR experience required
Must achieve a score of 85% or higher on required competency exams
Local candidates are welcome; same rates apply
Preferred Experience / Skills
Valid state license on file at time of submission preferred
Experience working in a fast-paced acute care or hospital environment
Familiarity with interdisciplinary team collaboration and complex discharge planning
