Required Qualifications
MSW or LCSW required
BLS certification required
Solid, demonstrated experience in care coordination
Prior experience working within the CA Dignity Health system required - candidates without this experience will not be considered
Preferred Experience / Skills
LCSW licensure preferred
Experience in an acute care or hospital-based social work environment
