Contract | Travel | 13-Week Assignment | Days, Monday-Friday 8:00 AM-4:30 PM | Rotating Weekends (Saturday & Sunday, 8:00 AM-4:30 PM)
Key Responsibilities

• Conduct face-to-face interviews with patients and families to assess social and discharge planning needs
• Perform InterQual reviews on all patients, including observation reviews and discharge planning evaluations (DPEs)
• Participate in multidisciplinary rounds (MDRs) on each unit daily
• Coordinate discharge planning and connect patients and families with appropriate community and area resources
• Float throughout the entire hospital, including Women & Children's and the Emergency Department as needed
• Document and manage case information using MediTech and Midas EMR platforms

Required Qualifications

• Licensed Master Social Worker (LMSW) - active and unrestricted license required
• Minimum 1 year of case management experience
• Proficiency with Midas and InterQual
• Ability to work rotating weekends (Saturday & Sunday); no on-call required

Preferred Experience / Skills

• Knowledge of local and regional community resources
• NATE certification or familiarity
• Outgoing, collaborative personality comfortable working across multiple units and teams
