First-time travelers are not accepted. Description: The Licensed Clinical Social Worker will provide support to patients and their families within the hospital setting. Responsibilities include conducting initial evaluations and coordinating care with internal and external team members. The social worker will collect and analyze patient information to facilitate care coordination. Documentation of care delivery, plans, goals, and pertinent information is essential to the role. In addition, the social worker will educate patients and caregivers to facilitate discharge planning. The environment may involve high-risk intervention scenarios, including suicide, homicide, and self-harm assessments. The patient demographic includes adolescents, adults, and geriatrics, with a daily census of approximately 150. Interdisciplinary support will include collaboration with various service teams. There is no requirement for floating or on-call duties. This position follows a Monday through Friday schedule with no weekend shifts.
