Key Responsibilities- Provide social work services within a fast-paced Emergency Department setting
- Conduct psychosocial assessments and coordinate discharge planning for ED patients
- Connect patients and families with appropriate community resources and support services
- Collaborate with interdisciplinary care teams to address patient social and emotional needs
- Participate in a rotating weekend schedule, every 4th weekend, with Friday and Monday off when the weekend is worked
Required Qualifications- Master of Social Work (MSW) or higher required
- Minimum of 2 years of social work experience
- Valid California State License required
- Must live within 50 miles of the facility to be considered a local candidate; local candidates are accepted
- Two professional references obtained within the last year required
- 7-year work history verification required
Preferred Experience / Skills- Experience with EPIC electronic health records system
- Prior experience in an Emergency Department or acute care setting
