Key Responsibilities
  • Provide hospital-based social work services to adult and geriatric patient populations
  • Apply crisis intervention techniques and critical thinking to support patient care and discharge planning
  • Collaborate with an on-site multidisciplinary team, including an on-site Program Director, to coordinate patient services
  • Utilize EPIC electronic health records system to document patient interactions and care plans
  • Demonstrate strong communication, time management, and problem-solving skills across daily caseload responsibilities
Required Qualifications
  • Master's Degree in Social Work required
  • Licensed Master Social Worker (LMSW) certification required
  • Minimum 1 year of social work experience
  • Proficiency with EPIC EHR system preferred or ability to learn quickly
  • Ability to work every other weekend and fulfill daily on-call requirements
Preferred Experience / Skills
  • Prior hospital-based social work experience
  • Experience working with adult and geriatric patient populations
  • Compassionate, empathetic, and adaptable personality well-suited for a fast-paced clinical environment
