Description

Key Responsibilities
  • Provide social work services across inpatient units including Med/Surg and Critical Care in a layered model working in tandem with the RN team
  • Manage discharge planning for a diverse patient population with a daily patient ratio of 8-20 based on acuity
  • Deliver counseling and social services support in a hospital acute care setting
  • Document patient interactions and care plans using Epic EMR
  • Participate in on-call responsibilities; subject to low census and call-back with a minimum 2-hour call-back requirement
Required Qualifications
  • Active Wisconsin Social Work License required
  • Minimum 1 year of experience in social services and counseling within a hospital acute care setting
  • Experience working with a diverse patient population
  • Proficiency with Epic EMR
  • Must live at least 50 miles from the facility to be eligible
Preferred Experience / Skills
  • Master's Degree in Social Work (MSW)
  • Experience in discharge planning within a multi-disciplinary team environment
  • Familiarity with Med/Surg and Critical Care patient populations
