Key Responsibilities
Provide social work services across inpatient units including Med/Surg and Critical Care in a layered model working in tandem with the RN team
Manage a patient caseload of 8-20 patients per day based on acuity
Support discharge planning and coordination for a diverse inpatient population
Provide counseling and social services support in a hospital acute care setting
Document and chart patient interactions using Epic EMR
Participate in on-call rotation; subject to low census and call-back with a minimum 2-hour call-back requirement
Required Qualifications
Active Wisconsin Social Work License required
Minimum 1 year of experience in social services and counseling within a hospital acute care setting
Experience working with a diverse patient population
Proficiency with Epic EMR
Must live at least 50 miles from the facility to be eligible
Preferred Experience / Skills
Master's Degree in Social Work (MSW)
Experience in discharge planning within an inpatient environment
Familiarity working within a collaborative, team-based care model alongside nursing staff
