Provide psychosocial assessments and social work interventions for hospitalized patients and their families.
Collaborate with physicians, nurses, case managers, and other members of the interdisciplinary healthcare team.
Assist with discharge planning, care coordination, and transitions of care.
Identify barriers to discharge and help connect patients with appropriate community and healthcare resources.
Provide counseling, emotional support, crisis intervention, and advocacy as appropriate.
Coordinate referrals to community agencies, rehabilitation facilities, skilled nursing facilities, behavioral health resources, and other services.
Maintain accurate and timely documentation in accordance with hospital policies and regulatory requirements.
Participate in interdisciplinary meetings and care planning.
Protect patient confidentiality and maintain compliance with applicable healthcare regulations.
