Key Responsibilities
Provide social work services within an acute care and med/surg environment
Collaborate with interdisciplinary teams to support patient care planning and discharge needs
Assess and address psychosocial needs of patients and their families
May be expected to float between nearby sister facilities as needed
Complete required onboarding modules prior to and during orientation
Required Qualifications
Minimum of 1 year of Social Work experience required; 2 years preferred
Acute Care and/or Med/Surg experience required
Valid state Social Work license; compact license must be in hand at time of submission
Must live more than 50 miles from the facility to be eligible at standard terms (locals considered at modified terms)
Must commit to working 3 of 5 designated holidays

2 professional references covering at least 1 year of experience within the last 3 years required prior to start
