Requirements:

• Required Certifications: MSW, LSW
• Required Skills: Assess response to treatment, Biopsychosocial assessment, Care coordination, Collaborative care, Collect and analyze patient information, Conduct initial evaluation, Coordinates with internal and external team members, Develop plan of care to meet patient/caregiver needs, Discharge Planning, Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documents patient response, Documents pertinent information to facilitate care coordination, Educates patient/caregiver to facilitate discharge, Establish and revise goals, Facilitate discharge and contingency planning, Participates in care conference activities, Plan of Care, Provide / reinforce individualized patient/caregiver education, Psychiatric Disorders, Refers to other disciplines/resources, Refers to rehab and long term care facilities, Review of Health history, 5150/ Involuntary holds, Complete intake and admission screenings, Family therapy, Group Therapy, Individual counseling, Participate in development of treatment plan, Perform Bio-psychosocial assessments, Provide patient and family education, support and advocacy, Psychiatric, Work with multi-disciplinary team to ensure continuity of care, Suicide/homicide/self-harm

Job Details:

• Pay: $1880/week - $2118/week
• Assignment Length: 13-week assignment
• Shift: 5, 7.5-Hour 08:00 - 16:00
• At least one year of experience required
