Job Title: Client Navigator Profession: Case Worker Specialty: - Duration: 8 Shift: - Hours per Shift: 35 Experience: Minimum of 3 years of experience, with a minimum of 2 years in health care or procedural scheduling License: - Certifications: - Must-Have: Bilingual English/Spanish (preferred) Description: Under supervision, provides social services to clients/patients. Identifies, develops, and implements social service plans for families/individuals. Determines eligibility for public assistance, food stamps, and medical assistance. May perform counseling and investigatory activities related to the work. Responsible for the maintenance of data and tracking of appointments and health outcomes. Facilitates care by coordinating outside and in-house referrals. Ensures the necessary information is gathered for complete patient care. Provides navigator support for newly diagnosed patients. Evaluates barriers to care. Refers patients to the genetic testing program by interviewing them. Processes financial hardship forms and communicates with the testing company. Maintains a patient database of all presenting patients. Responsible for maintaining compliance records and patient data. Facilitates the financial clearance and counseling process for patients. Monitors the tumor registry to identify relevant data. Maintains effective ongoing communication with clinicians and staff. Establishes and maintains relationships to ensure optimal patient experience. Identifies and participates in community outreach activities. Schedules and coordinates meetings with providers and administrators. Verifies insurance information and authorizations. Monitors referral work queues to triage and schedule patients. Enrolls patients in the patient management system. Reports issues of concern and provides recommendations.
