Travel Hospice RN – Case Manager
Job Details
Location: Sunnyvale, California
Start Date: November
⏳ Contract: 13 weeks
Shift: Days, 8:30 AM–5:00 PM (flexible based on patient availability)
⏱️ Hours: 40 hours per week
Setting: Hospice / Home Health
EMR: Homecare Homebase (HCHB)
Weekends: One weekend per month
Holiday Coverage: Required
Call: No call
️ Territory: Oakland, Alameda, San Leandro, Castro Valley, Hayward, Fremont, Milpitas, and surrounding cities
Requirements
Active California RN license
⏳ Minimum 3 years of Hospice RN experience
Current BLS/BCLS certification
Experience admitting hospice patients and conducting comprehensive patient assessments
Ability to develop individualized plans of care and coordinate with multidisciplinary teams
Strong patient and family communication, documentation, and follow-up skills
