Seeking RN – Hospice Case Manager for an 8-week contract in Montana.

Start Date: 9/16/2026 | Shift: Days | Monday–Friday, 8:00a–5:00p
Experience: 2+ years | EMR: Homecare Homebase (HCHB) | Positions: 2
License: Must be currently active
Responsibilities: Hospice nursing, patient admissions, case management, care coordination, patient/family education, and interdisciplinary collaboration.
Schedule: On-call and weekend rotation required. May start as a Visiting RN and transition to Case Manager.
Holiday: Required

PAY NEGOTIABLE
