POSITION SUMMARY – RN Case Manager – Hospice
POSITION DUTIES – Ability to manage patient care and provide services that require substantial, specialized bedside nursing skills for symptom management, crisis intervention and family education. In addition to excellent written/verbal communication, problem solving, and decision-making abilities, the candidate should demonstrate experience with developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team.
MINIMUM REQUIRED QUALIFICATIONS –
- Graduate from an accredited school of nursing
- Currently licensed to practice nursing by the Virginia State Board of Nursing
- BLS (AHA)
- Current Virginia driver’s license and driving record that meets the standards set forth in current policies
- 2 years experience in acute care, home health, hospice, med/surg or geriatric nursing
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – M-F 8:30a-5p
