Description

American Traveler is hiring a Home Hospice RN Case Manager requiring prior home hospice case management experience and EPIC charting proficiency.

Details

• Home health/hospice care setting
• Serves as Case Manager for a home hospice caseload
• Census of approximately 35-45 patients
• Day shift schedule, 5x8-hour shifts from 10:00 AM to 6:00 PM
• 13-week assignment length
• Uses EPIC charting system
• Floating may be requested based on facility needs

Requirements

• Active VA RN license or compact license in hand
• Current BLS certification through the American Heart Association (AHA)
• Prior home hospice case management experience required, no exceptions
• EPIC charting experience required
• Specialty experience within the last year required

Additional Information

• Manage a home hospice patient caseload, coordinating care and services
• Locals accepted at the same base rate
• Prior permanent employees must have been separated from the health system for at least 6 months before being eligible to travel
• Requests for time off should be minimal, ideally none during the assignment and no more than 7 days total
• All time off requests must be submitted with the initial application and confirmed in the offer; no additional requests will be approved after an offer is extended
