Description

American Traveler is hiring a Registered Nurse for a Hospice & Palliative Care position in a home health setting, requiring two years of hospice RN experience and an active NJ RN license.

Details

• Home hospice and palliative care setting with patients and families requiring end-of-life care
• Schedule of 5 days per week, 8a-4p shift
• Every other weekend required with comp days during the week
• Floating required to different counties as needed, including a nearby sister hospital
• No on-call responsibilities
• Uses Epic EMR for charting

Requirements

• Active NJ RN license or compact license
• Valid NJ driver's license required
• Current BLS certification
• Minimum two years of experience as an RN in a home hospice environment required, three or more years preferred
• Associate's (ASN) degree or diploma with NJ RN license required
• Experience with EMR charting required
• Strong critical thinking and communication skills required
• Knowledge of hospice regulations, pharmacology, and safe medication management

Additional Information

• Conducts patient assessments to determine hospice appropriateness and develops individualized care plans
• Performs admission, discharge, and re-visit documentation and data transfers within required timeframes
• Provides age-appropriate patient and family education and documents teaching tools used
• Manages caseload including care plan development, revision, and prioritization based on evidence-based practice
• Participates in interdisciplinary team (IDT) meetings every two weeks to communicate patient progress
• Maintains safe patient and work environment by following infection control and medication safety protocols
• Attends monthly staff meetings and participates in performance improvement (PI) activities
• Provides orientation and mentorship to new employees and cross-trains to learn new skills
• Local candidates accepted
• Requires background check and drug screening, with candidates expected to report to the lab within 24 hours of commitment
