Home Health RN – Vermont
Start Date: 11/17/2026
Duration: 20 weeks
Schedule: Monday–Friday Days
Weekend: Approximately every 4th week
EMR: Homecare Homebase (HCHB)
Seeking an experienced Home Health RN to provide individualized, goal-oriented nursing care in accordance with physician orders. The RN will also serve as a Case Manager when acting as the lead discipline.
Requirements:
Active RN license
Minimum 1 year of experience
BLS/BCLS
Home Health nursing experience
Strong assessment, care planning, and case management skills
Weekend rotation approximately every 4th week
