Description

American Traveler is hiring a Home Health RN Float requiring active MA licensure and certified home health case management experience for a 13-week assignment in the North Region.

Details

• Home health setting covering Chelsea, East Boston, Revere, and Winthrop
• Float position supporting North Region Scheduling
• Day shift schedule from 08:00-16:30
• Weekend rotation required once every 6 weeks
• Ability to travel to patient cases as assigned required
• Uses Epic for charting

Requirements

• Active MA RN license required (no pending or conditional licenses accepted)
• BLS certification required (AHA only)
• Minimum 1 year of experience specifically within a certified home health agency required
• Minimum 1 year of case management experience required
• Hospice, private care, or standalone case management experience does not meet the certified home health care requirement
• Epic EMR experience preferred
• Primary source license verification required within 30 days of start

Additional Information

• Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
• Perform admission processes and case management duties in accordance with home care policies and standards
• Maintain thorough, timely electronic documentation and interdisciplinary communication
• Flu vaccine required with no religious or medical exemptions accepted
• Local candidates residing within 60 miles of Boston are accepted at a reduced rate
• First-time travelers may be considered if they meet the experience requirements
