Key Responsibilities

• Complete OASIS documentation accurately and timely.
• Perform comprehensive patient and caregiver assessments.
• Develop and manage individualized plans of care.
• Serve as the case manager for assigned patients.
• Provide nursing interventions and high-tech care, including IV care, wound VAC, and complex wound care.
• Collaborate with LPNs and other healthcare professionals.
• Monitor and evaluate patient progress and response to care.
• Provide patient and caregiver education.
• Coordinate interdisciplinary services such as physical therapy and social work.
• Coordinate patient discharges and transfers.
• Communicate with physicians, providers, vendors, and other care team members.
• Conduct and participate in case conferences and peer-review audits.
• Advocate for patient and family needs.
• Maintain accurate clinical documentation and follow agency, state, and federal regulations.
