Job Highlights:
- Provide individualized, goal-oriented nursing care to patients in their homes.
- Manage patient caseloads and coordinate care as the lead discipline when applicable.
- Support and mentor a Nurse Resident in developing clinical skills and confidence.
- Work with Central Vermont Home Health & Hospice, a community-focused, not-for-profit healthcare organization.
- Mileage reimbursement at the IRS rate for travel between patients (commute excluded).
Requirements:
- Minimum 1 year of nursing experience.
- Active state RN license.
- BLS/BCLS certification.
- Home Health, Hospice, or Case Management experience preferred.
- EMR: Homecare Homebase (HCHB).
