Description

American Traveler is hiring a Home Health RN for a float role requiring active MA licensure and home care case management experience.

Details

  • Home health setting providing care in patients' homes across South Region communities including Allston, Belmont, Brighton, Brookline, Chestnut Hill, Newton Corner, Newtonville, Watertown, and West Newton
  • Patient population spans all ages from newborn to over 100 years old
  • Day shift schedule from 08:00 to 16:30
  • Weekend rotation required approximately once every 6 weeks
  • On-call responsibilities required according to department rotation
  • Uses Epic for charting
  • Uses UKG/Kronos for timekeeping

Requirements

  • Active MA RN license required with no conditional or pending licenses accepted
  • Both NURSYS and Board of Nursing verification required for license
  • Minimum 1 year of Certified Home Care experience required
  • Minimum 1 year of Case Management experience required
  • AHA certifications only
  • Flu vaccine required with no religious or medical exemptions accepted
  • Must have reliable, independent transportation to conduct home visits
  • 2 professional references required

Additional Information

  • Provide direct clinical care including patient assessments, care plan implementation, and ongoing education and support for patients and families
  • Perform admissions and case management duties in accordance with home health policies and standards of practice
  • Maintain thorough and timely electronic documentation and interdisciplinary communication
  • Local candidates may be considered if residing within 60 miles of Boston
  • No validated parking available at Boston locations; public transportation or paid public parking required
