Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Location: Albuquerque, NM 87109

Patient Care Setting: Home Health
Schedule: Full-Time | 8 Hours Per Day (40 Hours Per Week)
Travel: Local Travel Required

Position Overview:

  • We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Albuquerque service area.
  • The RN will be responsible for conducting comprehensive patient assessments, coordinating care, developing individualized care plans, delivering skilled nursing interventions, and collaborating with interdisciplinary healthcare teams to ensure exceptional patient outcomes.

Key Responsibilities:

  • Perform comprehensive home health assessments in both routine and emergent situations to identify physical, psychosocial, environmental, spiritual, and cultural patient needs.
  • Develop, implement, revise, and evaluate individualized Plans of Care (POC) based on patient needs and physician orders.
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and OASIS assessments.
  • Coordinate care with physicians, therapists, home health aides, caregivers, referral sources, and other healthcare professionals.
  • Deliver skilled nursing interventions including wound care, venipuncture, IV therapy, catheter care, medication administration, respiratory treatments, and disease management.
  • Assess and manage pain using appropriate pain management techniques while educating patients and families regarding symptom control.
  • Provide patient and caregiver education regarding medications, home safety, disease processes, diet, and self-care practices.
  • Supervise and provide clinical direction to Home Health Aides (HHA), LVNs/LPNs, and other support staff.
  • Maintain accurate, timely, and compliant clinical documentation within Homecare Homebase (HCHB).
  • Monitor cases to ensure compliance with payer requirements and agency standards.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
  • Perform on-call responsibilities as assigned.

Required Qualifications

  • Active Registered Nurse (RN) License in the State of New Mexico.
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma Program.
  • Minimum 1-2 years of RN clinical experience.
  • Minimum 2 years of Home Health experience.
  • Homecare Homebase (HCHB) experience required.
  • OASIS assessment experience required.
  • Current CPR Certification.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Strong clinical assessment, care planning, and patient education skills.
  • Ability to work independently in a field-based environment.
