Description

American Traveler is hiring a Home Health RN to serve as Patient Care Manager for a home health office, requiring SC RN licensure and combined general/home health nursing experience.

Details

• Office-based Home Health position managing a Patient Care Manager dashboard
• Adult to geriatric patient population
• Monday through Friday schedule with day shifts (8:00 AM - 4:30 PM)
• Responsibilities include reviewing orders, coordinating with physicians and clinicians, and processing new admissions
• May assist with scheduling and occasionally conduct visits outside the office
• Floating required within facility and scope of practice as needed
• On-call responsibilities approximately once per week as needed
• Weekend work as needed
• Mileage reimbursed at IRS standard rate and tracked through HCHB
• Uses HomeCare HomeBase (HCHB) charting system through 10/1/2026, transitioning to Epic thereafter

Requirements

• Active SC RN license or compact license required
• BLS certification required
• Minimum 2 years experience required (1 year general RN plus 1 year home health)
• HomeCare HomeBase (HCHB) charting experience required; candidates without experience must complete a paid online course prior to starting
• Experience with phlebotomy, IV management, and administration of blood/blood products preferred
• Experience with peripheral IVs, central lines, and ports preferred
• Wound care and urinary catheter management experience preferred
• OASIS documentation experience preferred
• Post total joint replacement and orthopedic care experience preferred
• First-time travelers accepted

Additional Information

• Serves as Patient Care Manager overseeing daily office operations and coordination of home health services
• Responsible for clinical assessment documentation, infection control precautions, and vital sign monitoring
• Scrubs of any color permitted
• Maximum RTO of 7 days per contract
• Holiday coverage required based on number of major holidays falling within the assignment period (Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Day, New Year's Eve/Day)
• Local candidates accepted at the same bill rate
